SMS Opt Out is not functional.
I checked the box for SMS Opt Out for a customer a long time ago, but found that he got a text from an automation. When I asked support about this, I got this response from support:
Unlike the Email opt-out field, the SMS opt-out does not actually unsubscribe the user from receiving further texts.
At the moment, it is just used as a field for internal reference and does not restrict or stop an automation.
Yes, I regret you would need to unenroll the contact manually or remove their number in case you wish to avoid sending further texts to them.
I have forwarded your feedback to the team as a suggestion for future upgrades.
